Bio/Background of the Speakers:

ConnOTA community of practice members are OT practitioners who are actively working in home care, who have worked in home care, or are seeking to work in home care.  Members bring their expertise and skillset as well as questions and a strong desire to use social learning theory to learn, share, and pool knowledge.

Topic: End Of Life Care Part 2

Learning Objectives Attendees Should Achieve:

This hour-long presentation and discussion will allow participants to:

1.      Expand of knowledge about end of life (EOL) care.

2.      Explore OTs’ role in hospice and palliative care.

3.      Understand reimbursement for hospice care.

4.    Share resources, knowledge, and experiences related to EOL. 

5.    Pool…those resources and knowledge and make them easily available to other CoP members.

Summary of Event: 

1. We will start with an informal check-in and welcome new members

2. Members will share their experiences related to EOL and OTs’ value.

3. Suggestions on how to obtain more referrals and in-service education on this topic will be explored.

3. We will continue to plan our topics and schedule every three weeks.
