Home Care CoP members

Bio/Background of the Speakers:

ConnOTA community of practice members are OT practitioners who are actively working in home care, who have worked in home care, or are seeking to work in home care.  Members bring their expertise and skillset as well as questions and a strong desire to use social learning theory to learn, share, and pool knowledge.

Learning Objectives Attendees Should Achieve:

This hour-long presentation and discussion will allow participants to:

1. List the top reasons why patients are re-hospitalized.

2. Understand Occupational Therapy’s value/role in preventing hospital re-admission. 

3. Learn about medication adherence scales.

4. Develop a medication “kit” that practically and functionally assesses patient understanding and ability. 

Summary of Event: 

1. We will start with an informal check-in and welcome new members

2. Members will share their experiences with medication management and OTs role to prevent re-hospitalization.

3. We will continue this discussion by planning for follow-up meetings on preventing re-hospitalization moving on from medication to diagnoses that are frequently re-hospitalized and what interventions OTPs can apply.
